TITLE:  Taking and Documenting a Comprehensive and Valuable Case History

ABSTRACT: This course will cover the importance of taking a thorough case history with proper documentation. Several videos accompany this course to demonstrate the good, the bad, and the ugly of doctor-patient interactions. Emphasis on Medicare regulations, with regard to coding and billing requirements, will be presented. All components of the history are presented with the rationale behind the questions, and the proper recording in the patient’s chart. Proper patient education by the paraoptometric will be covered as well, both during and after the examination.
